1. 2 duly-filled and signed by one or both parents “Travel Authorisation form for a minor“:
- When the absent parent is a Brazilian national residing abroad, his signature must be certified at the Consulate General of Brazil; a valid Brazilian passport or Identity card must be presented;
- When the absent parent is a foreign national residing abroad, his signature must be certified by a Notary Public in one original of the form, which must then be presented along with the other original (both duly filled and signed) to the Consulate General for legalisation;
- When the absent parent is in Brazil, he/she must send the other parent residing abroad a copy of the “Travel Authorisation form for a minor” duly filled and signed before the Brazilian Notary. The parent residing abroad shall sign the same document and submit it to the Consular authority in whose jurisdiction she/he resides.
2. 2 3x4cm actual photographs – 1 attached in each form
Note: the first copy of the form should be retained by the Federal Police agent on departure; a copy of the minor’;s passport or custody paper should be attached to it. The second copy should be kept by the minor or his/her travel companion. This service is free of charge.
Consular inscription
